Becoming a Patient

We will need basic information about you to set your first appointment.

As a valued patient at Open Arms Healthcare Center, you have certain rights and responsibilities. A copy of the Open Arms Patient Rights and Responsibilities Statement is provided to every new patient.


›  Personal Identification ( Driver’s License, State issued I.D., Student I.D.)

›  Your Health Insurance Card (if you have insurance)

›  Your co-payment (if applicable)

›  A list of medications that you are currently taking

›  If no insurance coverage, bring proof of income


Open Arms Healthcare Center accepts most insurances as well as offers sliding scale fees for our services for those with no insurance.

If you have private insurance, you may have a co-pay you must pay when you receive care. If you don’t have private insurance, our front desk clerk can help you with potential options. Payment is due at the time that services are rendered if applicable.

For patients with limited income, we offer a sliding fee schedule. Charges are as low as $25 for a doctor visit although additional charges for lab and other services may apply.

Payment plans are available with credit card payments.

No one will be denied services for lack of ability to pay.
